Join our dedicated team of healthcare professionals as a compassionate PT Home Health Therapist. Help patients in their homes by providing exceptional rehabilitation and therapy services tailored to their individual needs.
Key Responsibilities:
- Deliver in-home physical therapy services to patients, as prescribed by a physician
- Create and implement individualized, goal-oriented patient care plans
- Assess and document patient progress, adjusting treatment plans as needed
- Collaborate with physicians, nurses, and other healthcare professionals for coordinated patient care
- Educate patients and their families on exercise programs, injury prevention, and proper body mechanics
Requirements:
- Active state PT License
- 1-2 years of adult Home Health experience
- Familiarity with Home Care Home Base and Oasis documentation
- Ability to meet 30 productivity points per week
- Reliable transportation and valid driver's license
- Strong time management and ability to work independently
Preferred Qualifications:
- Experience with various patient diagnoses (neurological, orthopedic, cardiopulmonary)
- Excellent communication and interpersonal skills
- Proficiency with EMR systems
